$600 Wellness Credit – Due 9/30/2024

Earn the Wellness Credit to reduce your Aetna health plan rates by $600.

The deadline to earn the $600 Wellness Credit for 2024 was Dec. 15, 2023.

If you missed the deadline, you can still take action now to earn the $600 Wellness Credit for 2025.

Step 1: Get a complete physical before Sept. 30, 2024

Regular annual physical exams are an essential part of preventive care. They help find problems early when chances for treatment and cure are better.

Step 2: Download and complete the form

Print the Wellness Credit Form and ask your doctor to complete and sign it.

Step 3: Submit the form by Sept. 30, 2024

Step 4: Enroll in a 2025 Aetna Health Plan

The $600 credit will reduce your Aetna health plan rates over the 2025 plan year in each paycheck.

Please Note

• Participation in the Wellness Credit program is entirely voluntary
• No health information collected during the complete physical will be shared with Sodexo
• You do not waive the confidentiality of your health information in order to participate in the Wellness Credit program
• The Wellness Credit is not available for the following insurance plans:
  • Triple-S (Puerto Rico)
  • HMSA (Hawaii)
  • Kaiser HMO
  • COBRA
  • Retiree medical coverage
